@@karlmelrose Yes Honorary but is NOT the same as a board assigned title. Do some research and you'll find he allows himself to be referred to as Doctor Snowden and Dave Snowden PhD. His conjecture that some companies failed and some success while following the same processes. But he ignores the search for the Root Cause of both success or failure

@@GlenAlleman Thanks, but: 1. It says nothing about RCA in context of CAS 2. Do you think that for example the Ishikawa diagram or simple CAPA can be easy applied in the context of a Complex or Chaotic System? Do you assume a possibility (and costs) of the full understanding of all the feedback loops, emergence, butterfly effects, strange attractors, etc. to perform true RCA? 3. Who cares about RCA if a bottleneck is removed? (ToC)
