I think you forgot another one. In the ambulance challenge, James has an ambulance with a poorly built door. When he crashed onto a wall at reverse, the door opened, and the gurney with the patient is being dragged behind. As he reached to the hospital, the patient is gone.

I would have included hammond falling over in the snowmobile/combine harvester and (if i remember correctly) loosing his wedding ring(?) And also james cutting a hole in the side of jeremy's door and attaching it to the front of his volvo in the Africa source of the Nile special

Just because it's scripted doesn't mean it isn't funny. In fact, i think the fact we still can't tell what's scripted and what's genuine on this show shows how good the writing and production is
