It happens to me often in Fallout NW/F4, but due to guns simply being able to deliver death the moment i spot something. Like the most agregious one is in Fallout New Vegas honest heart, you get ambushed by tribals armed with ranged weapons and the moment you frag the last one, a friendly one (Who looks extremely similar to them at distance) charges to you. So he gets a bullet in the skull the moment he pops up and now i failed the entire questline of Honest Hearts. Atleast wave arms or something, or my itchy trigger might send you to the afterlife too.

If you had those legion guys as an enemy, I actually see how that could happen. Haha
