Sims didn't die because TS4 have a limit of how many simultaneous actions can happen. Basically, they were idle in a "queue" to catch fire, use the toilet, sleep and else.

Ah, I see you encountered the 'omg fire!' reaction. Sims are autonomously drawn to fire to panic, wave their arms and emote about it while pointing at the spot. The issue is they are drawn to the spot where fire first starts at, so sometime walk into areas where fire has spread allready. Another problem is sims4 is the WORST version to try to kill Sims in some ways. Starvation, fire and the usual older ways are harder to get to work than silly things like dying of embarrassment or laughing themselves to death. I think sims2 was best for death Sims events really, they started adding in protections in sims3, and went overboard with4 which is why I think they added in the mood deaths.

I’ve noticed with Sims 3 and 4 it’s harder to cause death. With Sims 1, death occurred very easily. You cooked a meal without experience, kitchen will probably catch fire and you’ll die. Cheap flammable items would often go up in flames more easily and cause death. A sim (often a child) plays with the pet guinea pig, they’d probably catch a disease and die. You forget all about one of your sims for a bit too long, their needs deplete quickly and they die. It was awesome, added to the challenge of really ‘looking after’ simulated reality

Do anyone.. know that video games have a clock speed as well as FPS. Every sim lowers the clock speed each time by 1/32 we will call this game memory. For a Sim to be random its has to join a "virtual memory input queue". Every other sim goes in that queue also. So for Sim 2 to move to eat, sim 1 has to read the action and to begin/finish reacting to the radio. When you have 100+ Sims every one of them have to get in the queue of random/given instructions and the game memory and CPU has to source that piece of code for the given instructions. If the CPU and game memory is getting 150 instructions at the same time of "reacting to being on fire" then the game clock slows down 150% putting pressure on the CPU and GPU essentially; regardless of how fancy a pc can be your gonna end up with clock speed hanging, causing the game clock to freeze "kinda like when you spam a computer mouse to the point where windows is being really slow" this is what's happening here. All of the Sims are stood in place because the CPU, GPU and game memory is trying to process all of the sent reactions at the same time causing graphical animation, function and memory hanging. This is essentially why all the Sims are frozen it because the game cannot handle it hence why we only get give 8 in a household it is so the CPU, GPU and game memory can keep up. When a modder gives instructions to not have more than 26 Sims it not optional it because they have tested the game with the mod so they had recommend the player to keep it at a certain level else the game will not work properly. The limit and setting are there to say the mod works up to this point..
