Kinda already did that concept in the first, would be a waste of the character.

They have done/hinted at the hallucination thing already, but they could spin that around, like we think she might be a hallucination, and even Joker think she's a hallucination, she's too perfect to be real. So he does as he wants with her, so sure she's not real he even mistreats her, only to find that she loves him for doing so, the fucking comedy that is attraction. It could be that she's the one thing that could really cure him, and she's the one cure the "doctors" took away forever. With a character this good and and actor as good as him, they could do almost anything.
