It’s estimated that 25% of people develop tolerance to their antidepressant medications. This medication tolerance issue was first noticed in the early 1980’s when we were using Monoamine Oxidase Inhibitors, called MAOis. These were very effective drugs, but you had to adhere to a really strict diet of no cheese and other things or else you could have a dangerous hypertensive crisis. Meaning your blood pressure could shoot up dangerously high.
Then in 1988 we got Prozac or fluoxetine as the first selective serotonin reuptake inhibitor. This was innovative because now we had another option that promised to have less side effects. But it didn’t take long before we started to see the same problem. Then the term was called Prozac poop out.
The classic way this looks is that you take the medication and get a full remission of your symptoms, meaning your symptoms resolve and you’re back to where you were before you started taking the medication. Then somewhere around 4 months to a year, you start feeling bad again even though you made no change to the medication.
The medical term for this is antidepressant tachyphylaxis. Which is defined as a rapid development of tolerance or immunity to the effects of a drug
Why isn’t this talked about more? Because it’s complicated and for a while, we weren’t sure if this was a true tolerance build up or if your return of symptoms was due to other reasons. Here are 5 other reasons you can have a change in your response to your medication.
1. Changing generic medication.
2. Not being on enough drug.
3. Having another medical issue.
4. You could be put on another medication that interferes with your antidepressant or causes depression.
5. Dopamine depletion
Here are 4 solutions.
1. Increase the dose of the medication if you can.
2. Decrease the dose or take a drug holiday
3. Change to an antidepressant that works differently
4. Your doctor can you give you an add on medication like aripiprazole or lithium.

Very good info. Bipolar2 for about 20 years. Frustrating when I get “stable” and then a few years later or even earlier a change in meds must take place. It’s always a road block that makes me feel like a failure. I get there then it seems to start to slip away. I am super aware of my moods and how to move forward with talk therapy or meds therapy. 20 years again. It teaches you so much if you really work the plan. Seeing this video has validated those emotions and set backs. It’s always a journey but never a real destination. Stay aware. Keep in constant contact with drs and work the program like your life depends on it.

@@Mscookie3131 agreed, there is no end destination like many other illnesses. Its updating that I have basically no hope for the future to be care-free/mental illness free like I wish it was. It sounds so nice to wake up everyday and not have any symptoms but unfortunately I can’t remember the last time that has happened

I ve been on 20mg Prozac (orodispersible) and beacause they stopped selling it in Ireland I had to go for a generic version and I feel a major difference.This is very helpful info! Thank you Dr.

Thx Tracey. Excellent video. I’ve been on SSRIs for 25 years (and also klonopin) for OCD, anxiety, depression and PTSD. They no longer really help me and all I feel is jittery and edgy. I’ll discuss maybe getting off them to see if my system resets. Take care!!

I've been medicated since 20 and I've found for me an average of 7 years is when they stop giving the same affect Bizzare but I backtracked in medical history and it seems that way

Thanks for this information, Dr. Tracey!! My mother in law started both Pristiq and Zyprexa for her bipolar disorder, and both worked for about 5 months, and suddently, she is starting to sink back into depression again, with her being very vigilant with taking her medications everyday. However, she also takes Metoprolol for HBP, and now it makes me wonder if this might have something to do with a possible tolerance. We are scheduling an earlier follow up appt. with her psychiatrist today.

Hi Lesly. We haven't see the same kind of effect with the mood stabilizers.I haven't seen this happen in practice either - at least from purely tolerance. I have seen the generic issue play a role in the mood stabilizers not working as well.

Hi Tracey I was diagnosed with bipolar II in April by a psychiatrist and my new psychologist thinks it might be triggered by displays of even the slightest anger from ongoing childhood trauma. Such a minor trigger is pretty much unavoidable so I’m getting triggered all the time and ending up really irritable, even aggressive and incredibly depressed. Could you please do a video on triggers and the interaction with bipolar? How do I get better from what could be complex PTSD? I just find the way you explain things so helpful! Thanks, Shev

I gave you a thumbs-up on this one. Generic formulations of the same drug can render them completely inert in some patients. The generic form of the drug might have an L on the molecule instead of an R.

Thanks for this information. Your videos are always well produced and accessible. You suggested bringing medication conflicts to your doctor’s attention. Should you stay with a doctor who did not notice a conflict with your medication? I don’t think I should have to notice conflicts and advise my doctor. I’ve had a doctor mess up my prescriptions. He’s at the top of his field as a specialist; however, he can be sloppy, careless, uncommunicative and unprofessional.

Unfortunately I think this happens a lot in the rush rush of medicine. That doesn't make it ok though. The combination of negative things you mention are a good reason to see a different person you can connect with better.

Thank you Dr Marks. A perfect question thoroughly answered. I've noticed that my 10 mg of Prozac- which I've been taking for years is no longer working. In fact I feel worse than before I started it.Other SSRIs like Celexa have side effects that make me feel worse and I always went back to the fluroxetine. Will be talking to my doctor today and your detailed explanations will help me focus on solutions. So glad you assume that your readers are intelligent human beings.

Hi Dr. Marx, I am currently working toward my LPC, and I was wondering if it is possible to have multiple mental disorders? I have seen some patients be diagnosed with as many as 4 disorders? In my coursework, I recall learning this is not possible, but in my professional work I’m being told the opposite. I love your videos and find them really helpful! I was wandering if you could do a video on forming a relationship/friendship and supporting someone with ptsd. I recently met someone who has severe ptsd from a previous abusive relationship (physical and mental) but we had to break it off because it got too much for her when she found out I had feelings for her. She is terrified of men and finds it hard establishing trust. She also suffers anxiety, depression, derealisation and has seizures. I would really like her to be apart of my life, even if it's just a friendship. How do I prove myself to her? I also suffer from severe anxiety which hasn't helped. Thankyou for any advice/help 🤗

Thank you dr. Tracey I have 2 questions please, Can treatment resistant depression happen to bipolar patients? Even when given for example Lamictal? Are there SSRIs or antidepressants considered contraindicated for bipolar patients? Even when given with a mood stabilizer?

Yes you can get treatment resistant depression with bipolar d/o, but we usually use that term for unipolar depression. If someone's depressed phase of bipolar is unresponsive, we think of the bipolar illness as treatment resistant. Great video, thank you. Speaking of MAOI’s, can you give your opinion on them? I was on Nardil and it took away all of my depression and anxiety, but within a month, everything went back to normal. What is your opinion on Tranylcypromine? How does it compare to amphetamine, or if it does at all? Does it work on dopamine? Thank you! Love your videos.

Thanks Jamie. I don’t have a lot of experience with the MAOIs because once the SSRIs came out they became unpopular. I think they work fairly well and some argue work a little better than the SSRIs. But since I don’t use them that much I can’t make that good of a comparison. The diet restrictions are definitely hard for people to maintain. The selegiline patch is better but I’ve seen people get rashes and not want to use it consistently. They don’t compare to amphetamines those are completely different medications. I don’t know if the dopamine enhancement occurs in the same location in the brain as what you get from the stimulants.

Hi Dr Marks, Thank you so much for uploading such excellent videos. Have you ever had anyone poop-out after 20 years? I did fine on 150mg venlafaxine for 20 years, but now it doesn’t work as good as it used to. I’ve switched back to my original brand, but it made no difference. We have increased the dose to 375mg and added 45mg of Mirtazapine, but I’m still not back to 100%. Have you ever experienced a really slow poop-out? (i.e. a 20 year poop-out?)

SS I feel your pain and want to encourage you to step out of the “normal” attitudes. You will get it. Over and over again. You will be stable and then need to readdress issues with pdoc. It’s never easy and always defeats you but to be the best you for those around you you have to keep working your plan

After 30+ different psych meds (since age 4 in 1971; I'm 55 now) ovr a 40+ yr period, I went psych med-free on 1/1/2017 (despite a Bipolar Disorder diagnosis). I've come 2 realize that mental health conditions (especially depression, schizophrenia, & Bipolar Disorder) r ENVIRONMENTAL IMBALANCES (NOT CHEMICAL IMBALANCES). In other words, it's how other people (mis)handle & (mis)treat u (especially when it involves abuse, bullying, & harassment).

Thanks for this video! Could you do a video on sleep issues in bipolar and how it differs to those without. Insomnia, hypersomnia, trouble waking up and delayed sleep onset disorder long term treatment. What medications can exacerbate those with both sleep apnea and insomnia. Does a normal psychiatrist tackle bipolar sleep irregularities or sleep psychiatrist?

"Normal" psychiatrists can tackle sleep issues. Usually people see sleep specialists if they have problems that require sleep testing like sleep apnea. There's also a cognitive behavioral therapy protocol for sleep that may be delivered by a psychologist or other sleep therapist. @@DrTraceyMarks I've been listening to the audiobook by Matthew Walker - Why we sleep. It had me asking lots of questions particularly if that applied to those with or without mental illness induced sleep issues. Also tried some of the light therapy suggestions in your other video and using blue light blockers etc. Aren't there risks of depression from long term use of melatonin.

What if ssri anti-depressants stop working over time because the brain starts to produce less serotonin in reaction to the drug trapping serotonin in the synapse. Plus the brain starts to down regulate serotonin receptors over time. So the brain eventually runs low on serotonin as a result. One possible solution may be to boost serotonin levels back up again by supplementing the body with amino acid tryptophan or 5-htp which will convert to serotonin.

Thanks for another great video. Prozac Poop Out! Sooo funny! It's even funnier because you said it with a straight face. :))) I tried Prozac and it gave me BLINDING headaches. I heard Prozac isn't prescribed much anymore because it gave a lot of people migraines. Is that true?

Ha Jan! I've probably said "poop" so many times it's not even a thing anymore 🙂 I still use a lot of prozac and haven't gotten migraine complaints. So I don't know about that issue.

It would sure be nice to have a Dr. work with me enough to be able to fine tune my meds. My community has 1.5 psychiatrists for an area larger than some countries and having a visit with them is lightning quick and if what you're on is working somewhat they say, "See you in 2-3 months!"

This is interesting! Im in the UK and I've been taking Lofepramine for about 3 years. Before taking it, I'd lash out, throw things, scream and felt depressed. This all started since giving birth to my first baby in January 2015. When I started Lofepramine in about 2017, this all stopped. It's now started up again and I'm on max dosage - 210mg a day. I have an appointment with an advanced nurse practitioner coming up and I'll ask about if I could be resistantto it now. I am also considering changing to a mood stabiliser which I discussed with my psychiatrist a couple of times since November 2019. She diagnosed me not long after with mixed personality disorder (traits of dependant and borderline)

Excellent information. I LOVE that you mentioned diet and how it can effect your moods. We are living in a nation that eats food-like substances and expect our brains to function normally. You can't supplement yourself into health! Eat the food!! Real food, as it comes out of the ground! I have bipolar disorder and learned from a good friend, that wrote a book about using nutrition to combat mental illness, that nutrition matters. I do like QSciences supplement for when I am going through a bad spell with the bipolar. But, it loses its effectiveness if I take it all the time. The real challenge is to remember to start it before I decend into what I call the outer darkness...LOL Gotta laugh about it, crying doesn't help! Thanks again.

I'm crying, crying for my sister who is just wasting away on all of these meds. WE DON'T KNOW WHAT TO DO FOR HER. I'm so afraid if we attempted to get her to taper off it will kill her. She's telling them she's not feeling any better (and I'm worried she's suicidal) so they just keep upping her doses, and she's like a zombie. She has her first grandchild on the way and can't even get excited about that. I will drive, fly or crawl carrying her to get her to this doctor.

Thank you for this great video, so if someone stops the antidepressant for about 4 weeks after being a few years on, would it be as effective as it was before? And what if you switch from SSRI to another SSRI, will it also work? Or it should be different class??

Hi Dr Marks. Can you do a video on if antidepressants ACTUALLY work? I’ve taken 3 and I just feel blegh all the time. I’ve been on each one for years at a time and find them to not be overtly helpful?

Great video. Very interesting and informative. Thank you so much Dr. Marks. I'm using Lyrica (pregabalin) for chronic pain for already about 5 years. I did take recently a medication holiday and stopped for about a few weeks in using this medicine. So when I started after that holiday again with this medicine it really does work much better again. I'm having less pains now. I think the brain gets a time out and can 'reset' again. It's like, when your computer is working slower after a time, you use a program to scan the PC well and after that it works faster again. So I think it's true, that my brain first got used to the medication and it didn't work not so well anymore after using it for 5 years. I like to do this medicine holiday time more often in the future. My question to you Dr. Marks is do you have any idea what might be a good time to take such a holiday? After one year maybe or so? Thanks a lot pocoeagle2. I don't know about a drug holiday for chronic pain conditions if that's what you meant. But for depression, I usually talk with people about taking a break from it after they've been on it a year and are doing well. Sometimes the same recommendation for anxiety if all is well and peaceful in their life.

Lyrica made me think about shooting up heroin, and I've never even been around it. That is a side effect of these very dangerous drugs. And we wonder how all these homeless addicts we see got that way. It's the damn vaccines.

I found this video very informative. I have experienced the loss of medication effectiveness. Also medications aren't happy pills. You can't be happy every day. antidepressants bring you back to a normal mood.

Can you do a video on building tolerance to adhd meds (mostly stimulants) I've been on vyvanse for nearly a year after taking a couple years off meds (having taken ritalin and concerta in school but getting nasty side effects). The vyvanse worked amazingly to start with but i think ive developed a tolerance because ive noticed myself falling into older less healthy patterns and becoming less focused

Yes that's definitely an issue. There's not a good answer to that though except taking a drug holiday. Same happens with benzodiazepines. I do have a video coming up on that.

Different brands of medication can make a difference. I take tramadol for pain and I've notice that some brands have a more stimulating effect while others a more calming effect. So now I keep the different brands for different points in the day, ie I dont use the more wakefulness brand at night.

Please be careful coming off on medications. Tolerance a given with prolonged use. Hope we have good results with less medication. Being free from XANAX has been liberating but I do have greater ups and downs. 💜👍🏽

I have a question, lithium destroyed my thyroid and I got taken off it, however lithium is the only thing that really helped me, antipsychotics gave me metabolic syndrome and I was told I cannot handle them. I've tried other mood stabilizers and am on lamictal 200mg but it isn't as effective for my mania or mixed episodes. I know that lithium can continue to affect my body and affect my kidneys, is it possible that I can go ahead and continue it despite this and take the risk? I would rather face renal failure or death and have a few more years of "stable" life that is healthy than more years that will instead be of poor quality and disastrous mood instability.

I've been on Mianserin for years, its never stopped working for me. It has been a real life changing medicine, as SSRI's never worked for me, maybe because I have ADHD.

Insurance won't cover brand name drugs sometimes even when it's medically necessary and not super expensive. They won't cover my wife's brand-name synthroid, for example. There was a huge difference in effectiveness from the generic not working seemingly at all, to the brand name dose needing to be reduced after switching.

I have a bad issue with developing tolerance for my anti-depressants. They last for generally around 6 months and that's about it. Sometimes, if I'm really lucky, it'll last for about a year, but I always develop a tolerance in the end. I've had depression for 15+ years now and I've honestly tried most of the different anti-depressant medications out there. I'm not sure what I'm going to do if I ever run out completely.

When I first started on Duloxetine it worked like a miracle for a little over a week. I started with 20 than 30 mg about a month later, and havn't felt different. I wonder what's going on. I also take seroquel 400mg and works great for sleep, no side effects and propanol 10mg for anxiety prn 3 times a day. I couldn't believe with that Duloxetine in the first week how well it worked, than quit. I eat very healthy and exercise, so I try, but struggle a lot. My diagnosis is schizoaffective disorder/depressive type. I'd love any feedback.

Ive been on and off Celexa, Adderall, and Xanax since 2012/2013 while switching out Celexa with different antidepressants along the way and currently i feel like I'm feeling WORSE than i ever have. The meds arent working and i feel like i am depressed anxious having insomnia and struggling to get out of bed and im just exhausted and last month because of the Adderall shortage i tried a generic ive NEVER tried before and it sent me into a literal Psychosis. I ALMOST checked myself in somewhere because i have a 3 year old daughter who needs me but thankfully it improved after a week but i got my normal brand of Adderall this month as long as my Celexa and Xanax and i just feel like im losing it or theres an interaction or something. i accidentally skip my celexa dose a lot (Thanks ADD) but last month i almost skipped half the month so when i got my antidepressant again this month i got side effects like i did when i FIRST started Celexa and it's been a NIGHTMARE. im so tempted just to throw everything out but i know i can't i am just tired of feeling like this and don't know what to do? Any advice is greatly appreciated!
