Dr Stephen Humphries of Harley Therapy discusses ADHD, including symptoms, effects on particular areas of the brain, different treatments, and medication successes
ADHD or Attention Deficit Hyperactivity Disorder, is a neuro development disorder that more commonly presents in childhood, but can and does persist into adulthood.
Dr Stephen Humphries FRC Psych is a Consultant Psychiatrist at Harley Therapy (www.harleytherapy.co.uk) Harley Street in London. In this video he talks about how ADHD is treated - including pyschical treatment, self-help, CBT and specialist drugs or medical help.
Although less widely diagnosed in adults in the UK than it is in the US or Canada, ADHD is a relatively common condition. Common symptoms can be anxiety, lack of confidence or depression, but standard treatments for these conditions can just make ADHD worse.
In our other videos, Dr Humphries discusses what kind of treatments are available for adult ADHD and how medication, in particular, can be spectacularly successful.

I’m crying inside. I’m thinking back to the young me trying so hard to “be better” at life and “be motivated”. I would have lists and lists of things to do from shopping, household chores to short-term and long-term life goals. I tried so hard and when I failed to stay on track I would hate myself so much and turn everything off to play a video game or read books straight through. I felt like I was such a disgusting, lazy and mentally WEAK person.I just want treatment now. I want to know how normal people feel, think and function!

When an expected reward fails to materialize we experience a drop in dopamine. That’s why you were going to video games etc to try and raise your dopamine levels back up. Try L-tyrosine (dopamine precursor) before going to stimulant drugs.

I have whiteboards with lists that are behind other junk and more whiteboards with lists on them. Never got around to even hanging one. I kept one flat on a desk. I have a grid of post-it notes where I lay my keys and wallet. Never read them until I’ve either accidentally done some of them or I’m just cleaning the area and go through them.

Don't want to sway you one way or the other, but I used to feel exactly like this and my life has completely changed from starting treatment with a stimulant (Elvanse). I don't take it all the time, and my GP prescribes my meds so that I can vary the dosage depending on what I feel I need that day, but just knowing that I can tune out the 'bad parts' of ADHD when I need to helps so much, and has helped me to appreciate the good parts so much more. Have you tried speaking to a psychiatrist about your options?

@@HeloisGevit I think it differs from person to person. For me, it's never running out of ideas, having seemingly unlimited energy when I'm excited about something, being amazing in a crisis (adrenaline is an incredible motivator, but for me also the world seems to slow down and make more sense when I have to think and act quickly), and never backing away from a challenge (an upside to having no concept of time is I never feel daunted by something needing a lot of time and effort). My inability to regulate my emotions goes both ways: I get super excited by good things, even tiny things like a text from a friend I wasn't expecting. Most people I know with ADHD are better at associative/ lateral thinking and are more creative than neuro-typical people. We're also quick thinkers and love to ping ideas around, which you notice when you talk to someone else with ADHD: suddenly you don't have to apologise for having six trains of thought running at the same because unlike a neuro-typical person, your ADHD friend can follow you on all of them. I would recommend a book called 'Delivered from Distraction': the author (who has ADHD) talks a lot about the importance of working from a strengths-based position rather than focussing on limiting the impact of 'the bad parts' of ADHD. I don't think it's helpful to think of your own brain as a malfunctioning neuro-typical brain: it's an incredible brain, and it can do lots of stuff, so learn how to use it to its full potential instead of trying to fit it into a mold that will never be right.

Yep. I procrastinate going to bed, then get up late. And then I can't find anything because I never put things in the same place twice it seems, and that makes me even later. So then I work late to make up for being late in and the cycle continues.

What's frustrating to me is that I learned a lot of coping mechanisms and how to get things done because I had great parents who held me responsible while also encouraging me to do what I needed to do. School and work were a game to me and I could get away with doing a fraction of the effort with a much higher return. Now that I'm an adult with a lot of the similar experience as others with ADD no one believes that I actually have it. In my graduate counseling program, after taking the test for it my instructor was shocked and questioned if I was honestly answering the questions on it. They said that they were surprised I do so well academically but I explained to them it was all a game to me. I didn't study for exams in one place, I would "study" for hours, 10 minutes outside in a park, 10 minutes in the library, 5 minutes in the student center. I would pace in my room, talking out loud. Type out things as though I was sending it in a message to other people .I would study in a group with one or two people, just listening and trying to come up with some joke or creative approach to catch them off guard. Now here I am, working in academia and seemingly unable to get anything really done. I come up with so many ideas, in meetings I can engage and do things. When my director gives me projects I will have them done in my mind, but can't actually bring myself to write them out. As I type this I have a report with spreadsheets of data, an example of the structure how I want the report formatted, and all I need to do to finish it it move the graph over and type out a narrative. It's not a fun game anymore. No challenge, no grades, no scores. When I went to get help several years ago, as mentioned in other places they wanted to treat me with depression. I've taken depression medicine before, it didn't help me with my intrinsic motivation, it just made me less anxious about it. So much so I stopped caring about numerous other things. All that to say, it's beyond frustrating. This "disorder" is more a cognition in conflict with culture in my opinion. We are pathologizing a powerful approach that people with ADD have by not giving them meaningful work. Non-ADD people (who come off as quite dull) try to force fit others into their systems and structures instead of teaching them how to use it like a skill.

Sorry you've been through this. One thing to always remember: 99% of people are incredibly dumb (even the intelligent ones) and will never be able to grasp or understand the kinds of connections and nuances that you find effortless. You may as well be speaking an alien language or from another galaxy. Don't rely on other people (they'll never have a clue) and try to find your own way.

" I would pace in my room, talking out loud." - this was the way I managed to memorise all the poems needed for an exam. I did this in a few hours, did okay in the exam then forgot them all after and didn't bother to revise them for my finals. I also lost my music literature notes and lost my place as the top student. Being consistent is tough, even as a 47 year old. Glad your parents helped you out. Mine thought I was lazy and were pretty angry with me when I started failing miserably from the change in structure and need to self regulate at University... where I spent 9 years across 4 faculties and 2 universities for one miserable degree.

You cant believe it at 35, I'm 56 and this has literally blown my mind while opening my eyes and makind the past 56 years finally make sense. Yes, ive heard all about adhd but in regards to kids. I knew adults had it too but to finally be able to say I'm one of those adults is so amazing I wanna get up and dance. Im also wishing my mom were still alive so I could show her this and say see mom I wasnt stupid like you always told me I was. She'd probably still find something I do stupid anyway but man it would make me feel better for a time!!

@@rytisbertulis3325 how true your words are. Its also comforting in a weird way to read all these comments and know we're not alone. To this day my mother's words haunt me to the point I now tell them to myself. Ofcourse most would say "To her credit back in the 60's-70's they didn't know about ADHD" well, thats a load of bs to a child who's biggest flaws were she couldn't sit still in church and couldn't get past basic math and for that was told how stupid she was and what a utter failure she turned out to be. She even had to write it in my baby book which I found and read at age 8. Her words told to me daily did more damage than ADHD ever could. I'm so thankful for platform's such as this one, as it just popped up in my recommendations and thank goodness something drew me to watch it, all the way through. It's as if they were speaking just to me. And the 56 yr old little girl in me has finally found a little bit of peace knowing it wasn't anything I could've done to stop it. I was made to believe that part of me was just lazy and other part just didn't care. Wow, oh what a glorious day this is for me to be able to release all the negativity drilled into me. Now, the search begins to find the right Dr to diagnose this and start me on a path of recovery. Hell, now I'm wishing I had all the money I've wasted over the years going to different counselors that not one ever suspected ADHD. I wish us all the very best in doing just that.

@@spalmer8398 I'm just guessing, that you live in US. So ADHD is at least being acknowledged where you are. I am from eastern EU, so even getting the diagnosis will probably be complicated :) Also I have three kids, two of which I suspect also having ADHD (the smallest one is only 2 months old, so too early to know). Raising ADHD kid is really a struggle. I also say hurtful things to my daughter sometimes and then I regret doing that. I try to explain to her, that we are both very impulsive and some things are not meant, they just happen. It's not easy, at least try to partially understand your mother. There is also a possibility that your mother also had ADHD and her emotional dysregulation was a result of that. We are all fighting our own battles... I often act and do things I don't mean, don't we all...?

I have unwittingly designed my life on crisis, adrenaline, physical activities, and caffeine to cope, do better, build a life. At 54, this has had other consequences as a result. My son got diagnosed with ADHD four years ago, and I have been researching the disorder to better support him. What has become more apparent as I learn more, and look back at my life experiences, is that I LIKELY have ADHD as well? I need to be accurately evaluated, and should seek some kind of treatment for myself.

@@bekaam1695 Not yet. Still managing my life with the coping skills at my disposal. I’m back at full time work, and trying to keep all the pieces moving. I have reached out to a friend with adult ADHD, and he gave me the number to his Psychiatrist. Just have to get organized enough to make the appointment! Not my strong suit :(

Not great? His primary concern is treating being able to function at work? Work is the one place where I can find focus (due to the high pressure). He felt that ADHD was more of current fad, and that my symptoms were more consistent with anxiety? His function being to prescribe medication…I don’t want to start anxiety medication. He suggested going to a private specialist, where they could do a more thorough evaluation. At my age, I have learned to work around the worst of my symptoms. My work choice, I feel has come directly out of NOT being able to function in a more word driven environment. Not sure what treatment can open up for me?

I'm just about to finish my PhD with which I struggled enormously, even more with my master thesis. But still I want so much to continue as an Academic, I am a geneticist and doing computational analyses helps me to hyperfocus. Shame that academia is not a very comfortable career for neurodivergent people :( I found out, that martial arts are very good type of sport, because I feel "encouraged" to actually train, though it happend to me in karate class that I was suddenly loosing focus during a fight or when learning new moves. Which ended with bruises. I just can't belive how did I live my 31 years of live without knowing that I had ADHD... I will still probably go through my PhD defence somehow (wish me luck, it is difficult to find an english-speaking psychiatrist here), but I hope to get the treatment to improve my life afterwards.

I have only realised in the last few weeks that I have struggled with adhd throughout my life. I’m now 57 . Daydreamed my way through primary school. Apathetic to learning was written on my secondary report. Often got removed from classes for talking through teacher demo’s. Sport was what got me through. I have butted into peoples conversations for as long as I care to remember, to the point they stop talking to me. I hyper focus on hobbies before I lose interest and move to the next one. I feel like a rabbit in headlights when I’m given forms to fill in (doctors or dentist etc). I procrastinate badly and then feel terrible because I’ve achieved so little. I find it so hard to listen to people and lose focus so quickly. Suffer with mood changes and low self esteem. If only it had been recognised when I was young. Bit late for me.

@@dr.shathaaboud9388 I've just discovered and been diagnosed at 70. Two days ago I moved up to 10mg of a stimulant, twice a day. Wutdafeezy's comment stood out to me. I copied it because it helped to know someone else feels like this. But I'm not sure I'm noticing much difference on the meds yet. Maybe a little, but I expected to feel excited by an amazing change, and I don't feel much of a difference. I'm so disheartened. Do you think I'll notice a change after a while; like perhaps noticing that I've been getting more things done?

@@Alex-zp9qj the medical treatment is a road of trial and patience with different types of stimulants and non stimulants or combinations of them also different dosing. It is never one stimulant fits all. Every single ADHD person is unique with all what he/she is. Also, what your expectations from the treatment is playing a crucial role in the choice and dosing of your treatment. Walk through this path with an experienced doctor. God bless you

I am so glad my therapist gave me the choice between taking medicine for adhd or bipolar because she wasn’t entirely sure which one it was but knew it was one of them. She said that I would know the first day whether or not I’d need to stop stimulants. I knew on day one it was adhd. The difference I felt was astronomical.

"some people respond to tiny doses" thank you for this bit of info. It seems 5mg of Elvanse, though it doesn't last very long, works really well for me, but I only tried it once so idk. I took 5mg and then about 2,5h later took another 5mg. I was on a roll just doing one task after the other and it was much easier for me to divert my focus when I needed to and then put it back where I wanted it to be. Usually that's irritating and tiring to do.

A medical professional saying that coffee can be toxic for people with ADHD, but nicotine is fine; is what I needed to hear. Even tiny amounts of caffeine give me terrible brain fog, & I have been very cautious about the possibility of taking stimulants for that reason, but to hear that I'm not the weird strange one is nice to hear.

hell no.. Caffeine is the worst... it doesn't help my ADHD but in turn it triggers my OCD (which I've with great difficulty controlled)

I know this comment is a month old but ... I don't have OCD so I can't know what your experience is, but I find coffee to be incredibly helpful for me. I just started the ADHD assessment process, so I've never been pharmaceutically medicated. Coffee helps me function on a daily basis in a similar way to what I read/hear about meds. Now if only caffeine didn't also increase heart risks....

You see, hyperactivity is expressed either externaly by movements or internaly by anxitey impulsvity anger ext.... Fitdging, hair picking ,nail biting doodling is in between So, one may experience all the symptomes but with different manifestations

I don’t resonate with the physical exercise part at all personally. Even when I took it really far and I was the best shape in my life, exercising was a second job, a tenuous chore. I just felt spent and damaged afterwards.

I'm really pissed because it seems my symptoms and specifically whatever is wrong with my dopamine disregulation has gotten worse (it can happen to non-ADHDers so ofc to ADHDers too) and I used to have enough in me to exercise and do stuff that helped and now I don't anymore. Why? Because of society! My parents put the breaks on my needlessly silly actions, not understanding how important they were. I think my ADHD is mild or moderate but got worse due to my phone/internet usage that started around 10 years ago. I think if I started treatment earlier I would've perhaps never needed physical treatment. But nowadays I want physical treatment _in order to_ get into habits such as exercising regularly and other things that help. I'm so ticked off that I actually had this going for me about 3 years sgo. 3 years! And now what, it's gonna take 1 or 2 to really get into those habits again AND ween off the stimulant. If I even get a stimulant, or a diagnosis! My evaluation got resceduled because I executive dysfunctioned on the questionaires so badly and then the night before the evaluation an emergency happened. I barely slept let alone did them. I have two more weeks. I'll be writing a lot of things down. But yeah I'm angry because I had no idea about the syndrom that is ADHD and no suspicion I had anything (just lots of shame! And thought I was a defect) but was doing all the right things to treat it. I was actually being so smart about it, so spot on. And why did it not work? SOCIETY. Bcs I "shouldn't need" this and that. And when I explain with sound logic and heavy arguments why it's the best thing to do - without even knowing about ADHD! - I was shut down because I was "making it more complicated than it is". I used fear for motivating myself to prepare for exam as I had issues in learning. So two days before exam I was doing things in a slow pace and lagging the timelines but the night before exam my anxiety and fear reaches at its height and I hardly finish revising the stuff just before sitting the exam. Though I got good grades but the pain I endured was awful as my fear make this whole process a matter of life and death for me. My slow learning was due to obsession for perfectionism, low self confidence and attention deficiency. I still suffer in similar manner in professional life and this puts me so low emotionally and gets depression and anxiety attacks. Do you think undiagnosed ADHD in children, can lead to constant high cortisol levels and or adrenal fatigue can be the root cause for autoimmune disease/disorders later into adulthood? (Such as Hashimoto's thyroiditis) Or do you think an undiagnosed or undiscovered autoimmune disease / disorder just exacerbates ADHD?

I've done the Ph.D. equivalent. The good doctor jokes about cramming the dissertation like its cramming for an exam, but I'm sorry to say I can specify more precisely what a procrastinatory Ph.D. timeframe actually entails (a three-four months long adrenaline rush due to a fear of support running out and not being employable otherwise). Not to say that's how long I was a Ph.D. candidate, but effectively little of value toward the dissertation was accomplished in the years prior to that. I could sincerely intend to start anything in good time (and I often did), but I could never focus properly if there wasn't the fear and adrenaline rush.

How do I go about getting treatment for this? I am seriously desperate, I procrastinate everything, getting ready for work, unpacking boxes and I moved a year ago! I am late everywhere I go!l I fight sleep knowing I'll be miserable the next morning!! Help!!

Would anti-depressant treatment have a higher success rates, if ADHD were diagnosed first and not co-morbid depressive illness? ie. a patient with undiagnosed ADHD presents as depressed and is treated for depression might skew data for anti-depressant treatment. Or is the data already adjusted for this? I ask as I have a later in life ADHD diagnosis but have received anti-depressant prescriptions in the past. I report that they may not have made me feel 'well' but it was enough at the time to feel different, which provided some respite until I got bored and stopped taking them.

I've watched a few videos from Dr Humpries on ADHD and in one of them he said that anti-depressant medication often makes ADHD worse.

@@MatWith0neT Yes I've heard that too. SSRI's interfere with Dopamine and exacerbate symptoms of ADHD. I have been misdiagnosed all my life and have only recently been diagnosed with ADHD and possible Autism at 49. Looking back this medications messed me and my life up BIG time.

I just heard today that medication is designed for the younger mind, but it's possible to trick the mind/medication into kicking in if you are older. I suggest you look at ADHD websites for guidance. Apparently also, after taking them for about ten years, the effect wears off. I don't know how accurate these statements are, but I'm going to research it for myself. At 70, I haven't got time to waste dithering around with ADHD any more. At least I know what I'm fighting now!

I have Severe ADHD and I’ve been in medicated for over 15 years now finally after doing my own research I am going to go back on them. I hope they help me out I’ve been using weed and nicotine as a self medication and I would love to stop

People with ADHD usually turn to nicotine and other stimuli to help them when they don't know they have ADHD. It sort of works but it's pretty bad since the effect is so low compared to actually having good stimulants like the typical ones you would get. I realized how bad my ADD/ADHD was when I got into school to become an engineer. For me it sucked since my nicotine and very deadline focused way of studying didn't work anymore. I did try Ritalin that I got from an acquaintance of mine just to try, that week worked better than most of my life. It's a no-brainer if you truly have ADHD, the ciggs and weed can't compare. It's like you unlock that 90% of the brain(figuratively) almost instantly.

There's a very long waiting list through the NHS atm, it's not too expensive to get a private assessment for ADHD

I saw Stephen at.the beginning of the year after many years of diognosed. I.e Cyclothymia, OCD, ASPD and undiagnosed for ten years taurettes. It was an awkward encounter on my behalf as I have shy bladder and at the time I was stunning and tics and pacing the room after a agonising year.or with g unmedicated or even being seen while having to work. He seems.anoyed at first and justified.becauss I come across hostile but that was just bad timing and I get nervous waiting so needed to pee but couldn't lol he handled himself in a professional way and told me straight alas he didn't know me well. I got sent back to my original place because I was to unwell with bipolar apparently well these all run co morbid with Autism..so it seems I will be seeing Stephen again and I apologise about last time and look forward to dealing with someone who has 40 years.experience..hopefully.time.i am.more regulated and can get the job done.

I wish what he said here about addiction/misuse and tolerance was more widely known. I find it really disheartening that my medication is a controlled substance. There are so many challenges surrounding the fact that it's controlled, that fall to the neuro- divergent person to solve. Excuse my not so politically correct example, but it's like a person who uses a wheelchair having to carry thier own ramp.

I wasn't diagnosed until I was 20 something. Only recently, in the past year or two, have I seriously been trying out ADHD medication like Adderall and Vyvanse. Now that I have the right dose, after being taken off of Adderall (because of my insurance thinking I needed to try something else) and put on Vyvanse, I can say it helps a ton. At the same time, being assigned female at birth, I think people assumed it was anything but. I was diagnosed with a mood disorder, and I honestly believe that if it is true, than it is mainly because of my ADHD. I believe I'm probably on the spectrum somewhere as well, but getting that diagnoses has been even harder. Now I'm stuck on an atypical anti-psychotic that I cannot sleep without. I'm on the lowest dose I've ever been, and still it is something I'm working on one day getting rid of completely. It is a drug that has many negative side effects, as is, and is no joke. I really wish that the doctor, who first prescribed this shit to me, at 14 (I'm 30 now), can go...kick rocks. I get not every doctor gets it right, but it's really fucked with my health. I'm in such a better place, at the lowest dose, too. Before they had me on the high end, at like 1200 mg a night. It was nuts.

I make lists because im procrastinating on the things i have to do. Making a list gives me extra procrastination time. That goes something like this : When im making my list i dont like the font. I try all 300 fonts in my computer but there all so ugly. So ill just go online ' for a sec ' to download a font. But they're all ugly or not free. So im now searching for a way to hack the program / site / font. And find more interesting software i also gonna try to hack or get illegally. Because thats just way more fun. 13 hours later, its 6 in de morning, i have 5000 fonts in my computer which i will never use because i will forget about them when i leave my computer. And i havent even made the list yet. And im not gonna because i dont want to deal with those stupid boring fonts again. So i write it down IF i can find a pen. If not ill use a kohl pencil or something. When i made the list i will never look at it again, because: - i immediately lose the list. - or i forget about it. - or i can not decipher mu own handwriting. 15mg pseudoephidrine plus coffee fruit exercise basketball 10min snaks alone building. Some. Exercise is bad. Or too much. Snuff works. Toix me. Weed helps microdose. Never tried speed. Ritalin sounds nice. I can't do anything
