So far, another topic potentially demonetized by UKposts. This is a shame because my team put a lot of work into this video, and I think that it is a topic that needs to be discussed. I agree.. Knowledge, information, education should not be discouraged. Yet... a lot of people can't handle truth. Because they don't like the truth, it goes against their.... 'believes'? Because it's possible, to use for example heroine and still have a fulfilling life and useful life without turning into an addict. Many won't agree with me and it's okay.. It's just a shame that instead of listening and trying to be understanding they tend to be judgemental with a fixed opinion about the matter.

It seems like a race for professionals to figure things out with this drug. Right now it’s most dominant in Philly. It’s in every bag of fentanyl. Heroin is not a thing anymore. Almost impossible to find. Anyhow the rumor you inject in one spot and it comes out another is completly false. It only seems to happen when you miss the slightest bit from your vein. Because Xylazine is such a vein constrictor it kills your veins fast. What’s going on and how the wounds are getting so large is addicts are using what’s called “shooting patches “. Because addicts veins are usually shot and because the Xylazine is is being found as much as 70/30% in each bag you don’t need to hit a vein. So addicts are shooting into open wounds. I deal with them everyday on the streets of Kensington. That mixed with homeless and poor hygiene is leading to really bad infections. So far no reversal agent and addicts on as much as 140mg of methadone are complaining it’s not working for the withdraw symptoms like it used to before Xylazine was present. Just like it does in animals it’s the same in humans. It’s almost like a robot that shuts down. Complete unconscious but they are still breathing fine. They wake up About a hour sometimes little longer and it’s like nothing happened they appear completly sober, just confused as to when and where exactly they went out cold. Vets are selling small vials for thousands of dollars. A 50ML bottle fetches them around 5 grand. If it’s the strong stuff the 300mg/mL the prices are insane.

As a veterinarian myself who uses xylazine like drugs (a2 agonists) daily when performing surgery or other sedation this is a terrible drug to abuse. Based on the dose sedation happens relatively quickly(+/- 5-15 mins when injected IM or near instantly when injected IV) and has strong CNS depressive effects as you explained in the video. One important side effect of a2 agonists in animals is vasoconstriction, to me this might be what is leading to those horrible skin leasions. Reduced bloodflow to the skin either causing ischemia or slowing your immune system from getting to an infection on time. This is usually worst in the extremities where most people seem to have lesions. For animals we have registered antagonists(antidotes) in the form of atipamezole (Sedastop, Antisedan etc.). These drugs work just as fast to stop the sedation in an animal when injected IM(5-15 mins). While there is some transfer over mucous membranes to me, a nasal spray like Narcan for opioids is not possible for xylazine(at least not at the moment and not to my knowledge). A2 agonists are also not considered to be the safest sedation options having a lot more side effects and are therefore not used in young, old or sick patients. Seeing how most drug addicts would fall in these groups the use of this drug is very dangerous.

I remember telling doctors and nurses about tranq many years ago bc you'd still be horrifically sick despite the detox meds. They blew us off assuming that we were trying to get extra. A lot of people including myself ended up crawling out of detox to find relief despite the obvious ramifications. I'm grateful the medical community is finally recognizing this poison being put in our poison. They need to adjust the standard detox protocols so they can try to mediate how devastating the detox is from that tranq. Good luck of you're still out there looking for the next one. The way they pick on their open wounds is horrifying, they often use the disgust people feel to beg for money along with the stench of their wounds. Its so horrible and getting out of it is so hard. I remember when I used to wait for the bus to go to work in the morning I would talk with a man, never knew his name but he used to take the same bus as me to visit his daughter. He was a recovering addict and he was so proud of himself because his daughter was allowing him to visit her becaue he was getting clean; I felt proud of him too. I nearly lost my leg due to a skin ulcer that reached my bone. It is so bad out there. You can smell rotting flesh from about 20 feet away. I would say like 80% of people have some sort of grotesque, ulcerated wound due to these drugs. The hospitals do not know how to properly detox individuals addicted to fentanyl analogues and xylazine. The preexisting protocol is not effective and people are literally dying because they’re so incredibly uncomfortable in the hospital. They leave AMA from local hospitals because the remedy to their illness is merely minutes away. We need to change how we detox patients so that we can treat their infections before they leave. It took me two years, in and out of hospitals, to get clean. Finally, a doctor properly medicated me. I will forever be grateful. I would have suffered a below-the-knee amputation without his compassionate, effective, and realistic approach. However, always narcan someone in the event of an overdose. These drugs have synergistic properties. Even if the tranquilizer is narcan-resistant, administer it. I’ve noticed that detox from tranq is very similar to detox from benzodiazepines and I’ve suffered seizures from both. I have never lost anyone to an overdose who has been promptly treated for their overdose. This is not a hopeless situation, but many, many, many changes need to be made. Everyone, stay safe. Edit: detox protocol needs to be modified to include a ketamine-fentanyl taper and comfort meds as needed. It is the only way to keep patients truly comfortable. Right now, hospitals can only start you on 25mg of methadone. Sometimes 30mg. It is virtually useless. I received 6mg of dilaudid every four hours and 4mg of klonopin with it. Without it, I was so sick. You really need quite the cocktail to make it through. Trust me, no one is drug-seeking when their bone is showing beneath a pit of black flesh. We want to get better. We want to stay. It’s hard to when you’re vomiting nonstop and choking on it. When you’re shaking constantly. Sweating through your clothing and saturating the bed sheets. People really need to know what is going on before it ends up in their backyard. It will eat your limbs away. I know a young man named TJ who lost both his legs and one arm. He’s no longer with us.

I use to use opioids a lot, up until my wife got pregnant with our first child. Using made me feel like I was Superman and made me literally feel like I could do anything. When I stopped, I literally went cold Turkey, and wasn’t until years later after talking with a medical professional that I found out how that was the wrong move to just quit so suddenly. After I put it down, I experienced severe abdominal pains, really bad diarrhea, depression, lost interest in just about everything and I had something happen that I thought was linked to something else turns out what I was experiencing was literally a extreme withdrawal symptom that could have killed me. The thing I experienced was literally like extreme anxiety. First time it happened I thought it was from the marijuana I had been using to kinda help me along, so I put that down also, but the incidents kept occurring. This is exactly what I experienced. I would first feel like I’m freezing, and I would feel as if every muscle in my body was seizing up, including my chest more accurately my heart, which would cause me to literally become so paranoid that my heart would go from what felt like 0 to 100. Every time I moved didn’t matter if it was a small moment or big movement I would shake uncontrollably, and it would be hard to breathe. I would sweat bad also. These episodes would last anywhere from 20 minutes to an hour or more. Sometimes I would even get a real bad headache. Sometimes the headache would come on first and sometimes it would come at the end of whatever was happening. I later explained this to a medical professional because I wanted to know what the he’ll-was happening to me. He literally said this. That I was experiencing major withdrawal. That the times I was like that I was literally at deaths door. I was told that at any point during these incidents that I could have experienced major heart failure and that I’m literally lucky to be alive. Now I knew I would experience withdrawal and that it would be scary and difficult, but I was not prepared for all that happened. I should’ve went and seen a medical professional when I decided to come off all the opioid painkillers I was using, because just flat out quitting literally could have killed me. I’m now 5 and a half years clean. I still have moments when my stomach wants to cramp and still have depressive episodes but not the major symptoms I had been experiencing. Feels good to be clean and back on track. Sorry for the long comment but this kinda hit home for me as a former user.

The worst thing the US ever did was to rip pharmaceutical opioids off the street like they did! And I'm glad you mentioned that opioid withdrawal can kill you. So many people believe its only withdrawal from alcohol and benzos that can, but we've had several people who died from opioid withdrawal come into the ER, one girl died while I was in rrhab with her, and babies die all the time from opioid withdrawal (both born and unborn).

I’ve been through the medical opioid crisis. I am 70 years old with chronic pain. The issue is that the withdrawal process is so long and so horrific, that many people can not make it through. I get why they don’t. The withdrawal Itself becomes overwhelming at some point. No sleep, no appetite or vomiting, night sweats, fevers, and for me; complete restlessness even though your dog tired was the worst and it goes on for over a month. In the end you are no longer looking for the release the meds give you, your only looking to survive the ‘sickness’ of withdrawing.

I know from experience that it feels like the withdrawal will never stop, but it does. Hang in there. My therapist helped me with a mantra I'd repeat when the worst of the withdrawals hit me: I can do this. I will get through this. I am healing. Surprisingly, those three little sentences gave me the strength to keep sobering up and it was the best choice I've made. Been sober now 8 years from opioids and 14 years from alcohol. Reach out for support if you're struggling. You're not alone, friend!

As a veterinarian, I use xylazine on patients everyday. I do, however, routinely use atropine or glycopyrolate to keep the heart rate and blood pressure supported.I am puzzled by the statement “there is no known reversal agent”. Yohimbine reverses it very nicely. If I want to wake a patient up, Yohimbine IV will wake them up in 1 to 3 minutes. Even IM Yohimbine works well. Also, I am puzzled by the tissue and skin necrosis in humans. I administer xylazine IM in fractious patients, and I have never seen sloughing. Even when given accidentally subcutaneously when attempting to administer it IV, I don’t get skin or tissue sloughing. I’m sure, though, that drug abusers aren’t exactly using aseptic technique. Also, human skin is supplied with blood in a different pattern than animal skin is. Human skin receives blood from subdermal supply evenly (largely) over the whole body. Animals have areas of skin supplied by regional arterial supply. Those factors plus possible interaction with heroin or fentanyl could be a factor as well. Having used it on patients ranging from kittens to horses and cattle, I am startled sometimes at how the dosing differs from species to species. A 40 lb dog can be sedated with 0.2cc of large animal xylazine (100mg/ml), but that same amount can drop a 1,000 lb cow on the ground. A horse requires 5cc for sedation. After 30 years of practice, I was always worried that it would someday be abused by humans. Sadly, that day has arrived.

I’m a social worker who specifically works with people who inject drugs. Tranq is very weird in humans, I had a guy who used it once, using “proper” harm reduction techniques with a new needle and cleaning the skin before injection and his skin still developed an ulcer, after one use! He was being seen by our doctor and even with wound care, the ulcer lasted weeks. Perhaps the street tranq is being manufactured differently by the cartels? Perhaps in unsanitary environments?

But are things like narcan even helping… all you’re doing is making it ok for them to keep maintaining their addiction. You’re prolonging their suffering and destruction. And the despicable destruction they cause. My mother, and father, was an addict and openly admits that she was enabled by all these stupid things. They just helped her do as many drugs as she could get because you gave her all the needles and kits she could possibly use and then when she inevitably OD’d every few days some sad sap comes along brings her back and sends her off for her next high. These people need locking up. If you have to reverse them overdosing you should be forced to section them or 5150 them. Or whatever your equivalent is. If they weren’t roaming the streets destroying communities *they wouldn’t even be able to keep overdosing!* You people don’t care about them or helping them. If you did you wouldn’t keep making the situation worse. There’s a reason medicine is top 5, out of ALL careers, to have the most literal psychopaths and sociopaths. Keep living up to the stereotype. You’re doing AWESOME. Awesome at destroying lives and society as a whole. You’re SUCH HEROES. Enjoy your shift enabling evil. Be proud. And at least be honest about what you’re doing. We see it. Just like we saw you destroy our lives because you were too cowardly to deal with the Carlsberg Cough. Then between whining about being SOO busy and what brave heroes you were you STILL had time to ignore patients to make your braindead twittwat dance videos. You people should have the decency to be ashamed of yourselves. You should be too ashamed of the destruction you caused to even admit you work in medicine. A LOT of people are going to snap one day. And you’ll have NO WHERE to hide from the people and their torches, pitchforks and rope. Lots and LOTS of rope. Don’t worry you won’t have to share. They’ll be enough for you ALL.

Thank you. This is a very informative segment. I'm a recovering IV drug addict and have been dealing with chronic wounds for years related to my extensive IV drug use. I stopped using before xylazine became prevalent, and my ulcers date back to 2018 and earlier so i dont think they're xylazine-induced. I'm really glad this stuff wasn't around when I was using. One wound on my left foot has been open for the majority of the last 5 years. I've been through 5 hospitals, 3 wound care centers, and at least 15-20 specialists over that time, with little lasting success. Unfortunately, by the time you've damaged your body to the point of developing wounds, the damage may be done, and even if you stop, you may be stuck with a semi-permanent chronic wound for years. Due to my extensive injecting, I have essentially no viable veins from shoulder to fingertips or thigh to ankle. The last several times I've had to have an IV put in, they were forced to use my jugular vein as there were no other options. For patients with extremely poor veins, it seems like IV antibiotics might be a challenge to administer, and I'm sure xylazine doesn't help..

@@newbornlog2789 It saved my life. Yes being tied to something sucks but look at this video, that's the alternative or even worse, death. Don't be hard on yourself, its counterproductive and take your time if you can. I was on it for about 10 years and now off for just under 8, it actually wasn't that bad but one trick I came up with that helped when weening off was to, and this is just an example: one day, a whole, next day 1/2, next day 3/4, next day 1/4, next day 1/2 and so on. The point of that is one day you are cutting back and yes it sucks but the next day you will take more so you have something to look forward to. You get a break on the "up" day which leaves you more ready for the next "down" day and then you repeat that constantly going down. Replace day with week or however you wish to structure it. Trust me one day you'll be off, you'll get this feeling of freedom that you could travel anywhere at the drop of a hat, do anything, anytime, and you will hardly believe it. Its worth it.

@@Nefville Same here- I would be dead without it for sure. The way I see buprenorphine is it gives you a tiny buffer between you and opiates to give you a a better chance at getting clean- the rest is on you. After you have changed your lifestyle and put more time between you and active addiction, it becomes easier to get off the buprenorphine (but not necessarily easy). The other thing I noticed is that taking buprenorphine every day is nothing like having to use everyday either- it is honestly the same for me as taking a vitamin each morning. I have no high associated with it, no ups and downs throughout the day, it really is a stable and normal daily life. That doesn't mean it isn't worth taking the step to try and get off of it though if you can and want to- just depends for each person.

These people are using it repeatedly and at uncontrolled dosage. In a veterinarian setting the dose would be given once for the desired procedure without adverse long term effects. Once you are caught in it, you have to reach your own personal lowest low before you are ready/capable of accepting the fact that you are no longer in control of your own life and thus willing to accept others to help you get back up. In my time I've seen several people die, simply because they could not accept that what they where doing was killing them.

Thanks for the info. I found myself on a click-bait driven rabbit trail into some of the darkest parts of society this morning. Curiosity demanded I learn what the heck is going on, and your video really covered this particular new drug problem in America really well. Terrifying. Its one of those topics I think I would rather stay blissfully unaware of, but its probably good to have some basic knowledge about it. Praying for all those dealing with these drug addictions, both the addicts and the professionals trying to help them.

@munguiamoon generally in a drug use context, blacking out refers to getting so high/sedated that one cannot remember where they were, what they were doing, or basically anything for a period of hours or days. Alcohol, benzos, and tranq are all known to cause blackouts involving loss of memory and control over oneself. It doesn't necessarily involve passing out/becoming unconscious but can overlap.

Dr Raynor, thank you for this very informative video on xylazine. I’m an old school Veterinary Nurse and yes we used xylazine for horses because it was reversible with Tolazoline, which is very important when dealing with horses, but we used it daily with cats. It was used in cats with ketamine as a safe intramuscular anaesthetic for knock out (always harder doing IV’s on cats with many claws and teeth) then they were intubated and maintained on gas anaesthesia and oxygen. It is crazy to me now that not only Ketamine has been out there as a street drug for a long time but now Xylazine as well. Is the necrosis occurring in humans because it’s it’s only meant for animal use? I wasn’t sure why because it wasn’t happening at the injection sites. We did not have any problems like that in either cats or horses. Dogs were given different anaesthetics. Is there any reason why the reversal agent used in animals - Tolazoline, can not be used or has not been tried in overdose cases in humans? Or is it because there are too many drugs in the person’s system at the time? I’m just curious more than anything? Again, I really appreciate the video and the information that you’ve put out. I think people need to understand this.
@jonransdell
@jonransdell 6 місяців тому
Kind of a tangent, but ketamine isn't just a street drug anymore! It's being used to treat various psychological and neurological conditions (in humans in case that wasn't clear). It's been approved by the FDA, but it's still (at least not yet) not something that a typical doctor would prescribe. People go to ketamine clinics, detail their condition to the clinic's doctor, and if the doctor decides to move forward with the ketamine therapy, the person then pays $2,000 or so for several sessions of ketamine infusions.
@DVNGXR
@DVNGXR 5 місяців тому
@@jonransdellketamine has been used on humans for its entire history. Although it became less commonly used because of the hallucinations it causes, its still used clinically in cases where people may have breathing problems but need serious pain relief or anesthesia. The majority of drugs that would fill those roles are sedatives which depress breathing, but ketamine is not and is used as a safe alternative in such instances.

@jojowynne233
@jojowynne233 5 місяців тому
@@jonransdell Hey Jon, sorry I’ve only just seen your reply. I knew Ketamine wasn’t just a street drug. When I was Vet nursing most of the drugs we used in surgery and for anaesthesia like Ketamine, were human drugs with the same scheduling. I’m in Australia and Ketamine is used here for the same things you mentioned as well some type of extended infusion for opiate withdrawal. A friend of mine dislocated his knee at home very badly. The paramedics could not move him at all due to the high pain level. A specialist ambulance was called to give him ketamine as pain relief and OMG he kept distorting his face and telling the specialist he had an “ugly mug” (face - Aussie slang) 😀 but he was good to go. I was more surprised about Xylazine finding its way on to the streets and then hearing there’s no reversal when I know there is. Edit: I always feel really bad for people in America when it comes to the cost of Healthcare. When you said $2K for the ketamine therapy. I’m in Queensland, if we ring an ambulance it’s free. If you go to a public hospital, everything is free. You get treated for free no matter what. Heart surgery, ruptured appendix broken bones etc. if you have Private health insurance and want to go to a Private hospital, the ambo is still free. You might have out of pocket expenses depending on your health cover and if you need surgery etc. I have private health insurance which covers top hospital benefits and top extras so dentists, physiotherapists, chiropractors, acupuncturists or any sort of allied health services.. For this I pay $208 per month. I’m also on a disability pension so Doctors visits, radiology, pathology etc are mostly free. 99% of the time. I was wondering how it compared to the USA on private Health?

Four years ago I was hooked on Ketamine. My habit had built up from my teens, all through my 20's and into my 30's. From the outside my life looked stable - I was married, working full time and happy. Yet I was doing 4-5 grams a day and 'topping up' in the bathroom at work, just to make it through shifts. Quitting K was the hardest thing I ever did and I have never looked back once. Now I look at these people and I know that quitting for them is worse than anything I ever went through and I feel deep compassion and sympathy. No one is immune to the drugs. If any of us lived their lives, we would be addicted too. I just wish more was being done to help these people.
