He went into the Avatar state in the second episode, and nothing happened then either. But at the Air Temple, that's when it had an effect. So if you know how to water bend and your ability to water bend is forever gone. It doesnt mean you cant air bend or fire bend. I think Zuko's transformation was intentionally supposed to be jarring. I always interpreted it as Zuko putting in way more effort than he's used to into being more positive, rather than it being an unconscious change in his behavior. Look at how he instantly reverts back when Azula shows up.

I think slapping someone to bring them back to their senses is one of those loopholes for kids cartoons about showing physical violence.
